Application for Services - An "Application for Services" form is available for download and must be completed. Please ensure that all fields on the application are filled in. We will not process samples if information is missing. Comment fields and the embedded "project description" document are optional.
Please fill in the "Peoplesoft project # (Dana Farber)" field if your funds are administered by DFCI; fill in the "Purchase order # (outside institutions)" field if your funds are not administered by DFCI.
Please send an electronic version of the completed form as an email enclosure to Sig Verselis. Include a printed copy of the completed form with your samples when dropping them off.
Sample drop-off - The Sample Drop-off room is now located in the "fish room" on the 3rd floor bridge of the Smith Building leading to the Jimmy Fund Building at the Dana-Farber Cancer Institute. For RNA samples, place your 1.5mL Eppendorf tubes (with the sample name written on the top, and your initials written on the side) along with your printed application form, into one of the coolers that is located in the bottom half of the -80°C freezer. Select the size cooler most appropriate for your number of samples. Place the cooler containing the samples and application form into the upper half of the -80°C freezer. Please do not write on the coolers, as the application form inside will provide us with your information. Samples that are left in any other type of box will NOT be transported to the Core. Our courier will only deliver the blue cooler boxes.
You will need a Dana-Farber ID card to enter the room. If you do not have one, please go to the Security desk in the lobby of the Dana building and request to be escorted to the "fish room". The phone number for security is 617-632-3131 (extension 2-3131 within DFCI).
Results - When available, results may be downloaded from a password-protected area of this website. You will be assigned a user name and password, if you do not have one already. More details are available on the Results page accessible from the menu above.
